•Click 
button to add new survey questions and the following screen appears.
•Select question type from Select Question Type dropdown list.
•Enter title of the question in Question Title field.
•Select Make this question required checkbox, if the question is to be made mandatory in the survey.
•Select Visible, if the question is to be kept visible for all. Else it will remain invisible.
•Enter different options against the question type you selected in the Options box for radio, checkbox, dropdown and radio (side by side) question types respectively.
•Provide maximum text length in characters for Text box and Text area question types.
•Click 
button to save the changes.
•Click 
button to go to cancel the process.
